Community Housing with Supports

Description

Community Housing with Supports is a Housing Supports program operated by the Canadian Mental Health Association (CMHA), receives funding from End Homelessness Winnipeg through its Reaching Home program. This Housing First initiative incorporates a harm reduction approach, providing comprehensive wrap-around services to ensure participants maintain stable living conditions.

The program is backed by a multi-disciplinary team consisting of three rehabilitation workers who act as primary case managers, an Occupational Therapist, a Housing Coordinator, a Peer Support worker, and a Community Support worker. This diverse team collaborates to deliver personalized support tailored to the unique needs of each participant.
