Siloam Mission's Medical Transitional Care Units (MTCU) aims to provide safe and stable transitional housing for individuals who would be facing homelessness post-hospitalization.
Residents may stay a minimum of 3 months but are able to stay as long as they need to recover and transition into permanent housing. The MTCU will offer 32 flexible occupancy beds. The facility will be inclusive and low-barrier, welcoming diverse populations including those struggling with addictions and mental health challenges. The MTCU will provide 24/7 support through a multidisciplinary team, offering comprehensive case management, healthcare, and support services. Collaborations with healthcare providers and community partners will ensure coordinated care and smooth transitions. The goal is to improve health outcomes, reduce hospital re-admissions, and facilitate the transition to permanent housing.
