Transitional Services

Description

Siloam Mission's Transitional Services projec tprovides Housing Placement (primary), Support Service (secondary) and Prevention (secondary) funding for 76 spots to support Siloam Mission's Housing First participants of the "Transitional Services "project. The funding contribution also supports 36 additional spots for other Housing Supports which provide intensive case management for its participants.

This project provides holistic case management services to individuals who have chronic or episodic experiences of homelessness to help them achieve housing stability and connections to supports in their community.

The Reaching Home support goes to the wages of the program manager, supervisor and 7 case workers.
