The Wiisocotatiwin/Assertive Community Treatment Program (ACT) is a Housing Support and Support Services initiative operated by Mount Carmel Clinic. It is designed for individuals dealing with complex trauma, serious mental health issues, and substance use concerns. This Housing First approach is tailored for those who have not found success with traditional outpatient programs.
In collaboration with the Winnipeg Regional Health Authority, the program uses an Assertive Case Management model to provide comprehensive, community-based wrap-around services. This multidisciplinary team approach ensures continuous, responsive care that addresses the complex needs of participants, facilitating a stable and supportive recovery process.
Key services of the ACT program include housing placement and supports designed to foster economic, social, and community integration. This holistic method not only meets immediate housing needs but also promotes long-term wellness and helps participants forge meaningful connections within their communities, enhancing their overall quality of life.
