Description

The Youth Resource Centre System Navigation Program (SNP) is a Housing Supports, Prevention/Shelter Diversion, and Support Services project operated by The Link. The project is designed to assist youth aged 12-29 who are currently unhoused or at risk of becoming unhoused by offering comprehensive, barrier-free wrap-around supports.

The SNP provides a range of crucial services to support these youth, including advocacy, assistance with accessing housing resources, EIA support, help with obtaining identification and opening bank accounts, and accessing medical and mental health supports. Additionally, the program offers harm reduction, life skill building, addiction supports, crisis support, and daily necessities such as access to phone, computer, shower/hygiene facilities, and food at their drop-in center located at 175 Mayfair Ave, open six days per week.

The program offers both short-term crisis supports and long-term case planning through a person-centered, trauma-informed, and culturally informed framework. Operating under a rapid re-housing model, the SNP aims to swiftly move youth from homelessness to stable housing while providing continuous support to navigate the complex barriers they face, ensuring they receive the tailored help needed to secure and maintain housing stability.
